each item's Point Value.
When
you have finished filling out the form, click the

If you make any additional changes, click the "Calculate"
button again to update the Total Point Value.

records.
STEP 3: Fill out the required fields at the bottom
of the page.
STEP 4: Send the information to us by clicking
